Experimental Study on Flap Survival with the Heparinized Whole Blood Perfusion during Ischemic Storage


Abstract
Abstract-This study was designed to investigate the use of heparinized whole blood as a perfusate for extending the tolerable ischemic interval of free flaps.
Unilateral free groin flaps stored at 20¡Éfor 12hours were used in four groups of 20 rats respectively. One group served as a control perfusion of normal saline while a second and third involved perfusion of lactated Ringer's and balanced salt
solution.
The fourth group underwent a perfusion of heparinized whole blood mid way through the ischemic interval(12hours).
After the 12hours interval, flaps were replanted to the original groin site with standard microvascular technique. Percentage of numbers of surviving flaps were : 10 percent for controls, 55 percent for heparinized LR perfusate group, 65 percent
for
heparinized BSS perfusate group, 90% for heparinized whole blood group. With lengthy ischemic-storage involvement as with extremity amputations, periodic perfusion with oxygenated, anticoagulated blood may enhance probability for as favorable
outcome.
